Passport-size photographs of the child: You should bring several recent photographs of the child as required by the PSK. Declaration form (Annexure D): Ensure that both parents sign the declaration form, which is available at the PSK. This form is a crucial part of the application process. Address proof: Provide a proof of current residential address. This can be a utility bill, rent agreement, or any other document that shows the address under either the father or the mother.
