When ingested, methanol undergoes metabolism to form formaldehyde and formic acid, both of which are highly toxic substances. These metabolites can cause metabolic acidosis and inflict damage on the optic nerve, resulting in blindness. The severity of methanol poisoning can vary widely based on individual health factors, the presence of other substances, and the amount ingested. Even small amounts can be incredibly dangerous, underscoring the importance of immediate medical attention if methanol ingestion is suspected.

Historically, methanol poisoning has been associated with illegal distillations during the Prohibition era in the United States when moonshine was produced clandestinely. These homemade concoctions sometimes contained methanol as an accidental contaminant, leading to several cases of large-scale blindness. Even today, denatured alcohol, which is essentially ethanol contaminated with methanol to make it undrinkable, is sold in hardware and drugstores. The intention is to prevent people from drinking it for recreational purposes, but the presence of methanol still poses a significant risk.

Preventative Measures and Common Household Dangers

To avoid the life-altering consequences of methanol poisoning, it is vital to keep all methanol-based products, particularly windshield wiper fluid, away from children, pets, and any household members without proper knowledge of the dangers. Decoration paints, industrial solvents, and antifreeze are other substances that should be stored safely to prevent accidental ingestion.

The Metabolic Pathway and Its Effects

When ingested, methanol undergoes a metabolic pathway that results in the formation of formic acid, preferably with formaldehyde as an intermediate. Both these compounds have the potential to denature proteins, with a particular affinity towards the proteins in the optic nerve. A mere 10 mL of ingested methanol can therefore lead to permanent blindness. In more severe cases, 15 to 20 mL can be fatal.

It is also noteworthy that even topical application of 10 mL of methanol to the eyes can have devastating effects.
